I saw the bill on my credit card statement and it was $35 instead of my usual $10 monthly bill. So, I cancelled it and went over to my local planet fitness branch in Great Neck. I was told the $35 included my annual fee. I believe you should state that on the credit card bill. Anyway, I went back and cancelled the cancellation and hope that everything is now in order. AGAIN, in the futre, when you send a bill, it should state monthly fee AND annual charge.
